By reading and submitting your client intake form, you are agreeing to the following terms and conditions set out by Aesthetic Affairs. These terms are intended to ensure clarity, safety and professionalism between practitioner and client. Please read carefully and ensure you understand before providing consent.
All treatments provided at Aesthetic Affairs, whether aesthetic, beauty or otherwise, are carried out by qualified practitioners. Treatments may include but are not limited to injectable procedures, skin treatments, fat dissolving, beauty therapies and cosmetic dental services such as cosmetic teeth whitening. While every effort is made to ensure safe and effective results, all procedures carry inherent risks, including but not limited to swelling, bruising, sensitivity and potential adverse reactions. By reading and submitting your client intake form, you acknowledge that you have been informed of possible risks and outcomes, and that no treatment can be guaranteed to produce a specific or permanent result.
It is the responsibility of the client to provide accurate and complete information on their medical history, allergies, medications and any relevant conditions. Failure to disclose accurate information may increase the risk of complications, for which Aesthetic Affairs cannot be held liable. Clients must inform the practitioner of any recent or ongoing illnesses, infections, skin conditions or dental concerns, as well as any medical procedures or treatments they are currently undergoing. Pregnant or breastfeeding clients are not eligible for most treatments, and it is the responsibility of the client to declare this information. Clients under the age of 18 will not be treated under any circumstances and proof of age may be required.
Clients should understand that aesthetic and beauty treatments are elective and cosmetic in nature. Results vary between individuals and depend on factors such as skin type, lifestyle, aftercare and adherence to practitioner instructions. Maintenance sessions may be required for optimal or ongoing results. Clients are advised that photographs or videos may be taken for medical record purposes, and where model slots or discounted treatments are undertaken, images may also be used for educational or marketing purposes. Photographs are non-negotiable for insurance purposes, and we retain the right to decline treatment if this is declined by the client.
Aftercare is a vital part of the treatment process, and clients must follow the guidance provided by the practitioner. Failure to follow aftercare instructions may negatively impact results and increase the likelihood of complications. Should a client experience an unexpected reaction or concern following treatment, they must contact Aesthetic Affairs at the earliest opportunity. Emergency aftercare and advice are always available.
Cancellations and rescheduling must be made within the timeframe outlined in the booking policy. Late arrivals or missed appointments may result in loss of the booking fee or treatment fees. Booking fees are non-refundable and Aesthetic Affairs reserves the right to refuse treatment where it is not deemed appropriate, safe or in the client’s best interests.
All clients must understand that treatments do not replace medical care, diagnosis or treatment by a healthcare professional. Any concerns of a medical nature should be directed to your GP or relevant healthcare provider. Cosmetic teeth whitening, where offered, is a non-medical, cosmetic service. It is not a substitute for dental treatment and should not be undertaken if there are untreated dental issues, cavities or gum disease. Clients are responsible for ensuring they are dentally fit prior to undergoing any cosmetic dental service.
Treatments are secured with a 50% booking fee and the remaining can be paid via cash or card on the day. Promotional offers, discounts and memberships are subject to change at the discretion of Aesthetic Affairs. Where referral codes or discount codes are offered, these must be used in accordance with the stated terms and within the timeframe given. Gift cards are non-refundable and must be redeemed within their validity period.
By reading and submitting your client intake form, the client confirms that they understand the nature of the treatment(s), have disclosed all necessary medical and personal information and consent to proceed with treatment at their own risk. The client also confirms they have had the opportunity to ask questions and that these have been answered satisfactorily. Aesthetic Affairs accepts no liability for outcomes that do not meet personal expectations, as results cannot be guaranteed.
