Terms and Conditions 1. Introduction Welcome to Eternal Beauty Aesthetics. By booking an appointment with me, you agree to the following terms and conditions. Please read them carefully. 2. Eligibility • You must be 18 years or older to receive treatments. • You must provide valid identification upon request. • You must paid the non-refundable booking fee MUST be paid at the point of booking, failure to do so will result in your appointment being cancelled. • You must complete the Medical Form within the next 24 hours. • You must complete the Consent Forms within the next 24 hours. • Any changes in your medical circumstances must be communicated immediately. • Eternal Beauty Aesthetics is not liable if your forms are incorrect at the time of treatment. • You must state any medical conditions and medications on your forms. • By completing the forms, you agree that all information is accurate. 3. Booking • Appointments must be booked at least 48 hours in advance via website, phone (07715301763), or email (eternalbeautyaestheticsbyloran@gmail.com). • A 50% non-refundable booking fee is required at the time of booking and will be deducted from your final cost. • The 50% non refundable booking fee MUST be paid at the point of booking, failure to do so will result in your appointment being cancelled.. YOUR APPOINTMENT IS NOT SECURED UNLESS YOU PAY YOUR NON REFUNDABLE BOOKING FEE, failure to pay the booking fee will result in your appointment being cancelled. • The remaining balance is due immediately after your appointment. Payment methods include credit/debit cards. NO CASH ACCEPTED. 4. Non-refundable Booking Fees and Payments • 50% non-refundable booking fee is to be paid on all treatments, at the point of booking. The remaining balance to be paid immediately after treatment. Failure to pay your book fee at the point of booking will result in your appointment being cancelled. 5. Cancellation, Rescheduling, Late Arrival or No Shows Appointments: • If you need to cancel your appointment, please provide at least 48 hours notice, failure to do this will result in your card being automatically charged for the remaining 50% balance of your treatment. • If you need to reschedule, please provide at least 24 hours notice, failure to do this will result in your card being automatically charged for the remaining 50% balance of your treatment. • If you’re 15 minutes or more late to your appointment, I will be unable to carry out your treatment and you’ll still be required to pay the remaining 50% balance of your treatment/s. This means that your card will automatically be charged for the remaining 50% balance of your treatment. • If you do not show up to your appointment, your card will automatically be charged for the remaining 50% balance of your treatment. 6. Health and Safety • Disclose all relevant medical history, allergies, and medications before treatment. • Notify me of any changes in medical conditions immediately. • I adhere to strict health and safety protocols, sterilizing all equipment. 7. Mental Health Disorders and Aesthetics • Clients should have realistic expectations focused on improvement. • Discuss any significant emotional or mental health history prior to treatment. • Psychological benefits are possible, but not guaranteed. 8. Limitation of Liability • Myself Loran Robinson, or Eternal Beauty Aesthetics are not liable for economic loss or any indirect or consequential loss. • It is your responsibility to provide accurate medical details. • You must follow all aftercare instructions provided. 9. Risk of Clinical Complications • No clinical procedure is entirely risk-free. • Complications, though rare, can occur and will be discussed prior to treatment and is covered in your consent forms. • Contact me immediately if you experience complications. • Serious complications may require referral to the NHS or private treatment at your own cost. 10. Liability • Myself Loran Robinson, or Eternal Beauty Aesthetics are not liable for adverse reactions from undisclosed medical conditions. • Myself Loran Robinson, or Eternal Beauty Aesthetics are not liable for negative results if aftercare instructions are not followed. • Myself Loran Robinson, or Eternal Beauty Aesthetics does not accept any liability in contract or in tort for any injury, damage, loss, delay, additional expenses or inconvenience caused directly or indirectly by unanticipated or uncontrollable events, or directly and indirectly in connection to any aesthetic treatments carried out by [loran robinson] and [eternal beauty aesthetics]. • By accepting these Terms & Conditions you agree to release, defend and hold harmless [loran robinson] and [eternal beauty aesthetics] from all such liability and indemnify each of them severally against such claims that may be made by or through you relating to your breach of this agreement. • Contact me within 7 days of treatment for any complaints. • I am insured by PolicyBee, valid until May 2025. 11. Privacy • Client information is confidential and used only for providing and improving treatments. • Personal data is not shared with third parties without consent, except as required by law. • If you do not want your before and after photos used on social media, please let us know. 12. Changes to Terms & Conditions • I reserve the right to update these terms and conditions at any time. Clients will be notified of significant changes. 13. Governing Law • These terms and conditions are governed by the laws of England & Wales.
