You understand that a patch test for eyelash glue is available upon request; however, a patch test for extensions cannot reflect the amount of glue in comparison to a full set. A patch test for brow and lash tinting & lifting is compulsory. That the service provider, Julia Scott, is not legally liable for reactions/irritation/discomfort that may occur on rare occasions.
That this agreement will remain in effect for this procedure and all future follow-ups conducted by the certified eyelash technician. I have read and fully understood all of the information in this agreement. I am over 16 years of age (or are a parent/legal guardian and consent to the agreement and to the eyelash extension procedure).
