Terms & Conditions of booking in with Illusive Artistry Upon booking in you accept the terms and conditions as set out in my policies within the policy section which is viewable on my website. Which includes all policies and the Cancellation & FTA policy. If you are booking a treatment for the first time a mandatory consultation form must be filled in prior to your appointment this is sent via email once the booking has been accepted, failure to complete this will result in the booking being cancelled and a 25% cancellation fee being charged for cancellations for failure to complete the consultation form. This is to protect not only my business but to be compliant with insurance. Thank you for understanding
