I understand the importance of informing my therapist prior to treatment of all known allergies, medical conditions and medications I am taking, and to let my therapist know prior to treatment, about any changes to my health.
I understand that my therapist may request a doctor's note prior to treatment if necessary and if a doctor's note is NOT presented then my therapist can refuse treatment and a refund will be processed if necessary.
I understand that contra-actions may occur due to beauty treatments and if any irritation or contra-actions should occur to any products or practices used during treatments, I should inform my therapist ASAP and consult my GP if necessary.
For Eyebrow/Eyelash Tinting and Lash Lift and Brow Lamination Eye Treatments; I understand that a patch test will need to be carried out at least 48 hours prior to treatment. If I refuse a patch test for any reason then my therapist can refuse treatment and a refund will be processed if necessary.
Eyebrow/Eyelash Tinting, Lash Lift and Brow Lamination treatments can only be carried out for persons aged 16 and over. If I book to have these treatments, I am confirming I am aged 16 or over.
Body treatments can only be provided for persons aged 18 and over. If I book to have a body treatment, I am confirming I am aged 18 or over.
Gel polish application is not recommended for children under the age of 16. Young children's nails are thinner and more delicate than adult nails, making them more susceptible to damage. Therefore, anyone under the age of 13 in unable to book a gel polish/builder gel treatment. However, if any person is aged 15 - 13 and would like to book a treatment including gel polish/builder gel, parental consent via email to petalsbeautyroom@outlook.com is required prior to treatment. If parental consent is NOT presented prior to treatment, I understand that my therapist can refuse treatment and a refund will be processed if necessary.
