Cosmetic Tattooing:

I declare I am 18 or above.

Patch tests are highly recommended. Please note patch tests are sent out 1 week prior to your scheduled appointment, so please book in time for this to be sent out. However, walk ins are welcome.

I understand that it’s my responsibility to request a patch test before my scheduled appointment, If I decide not to undergo a patch test and a reaction occurs, it’s not at the thought of Rose PMU Studio and excludes all legal involvement.

Please do not book an appointment if you are pregnant or breastfeeding, epileptic, suffer from lupus, have a pacemaker or any major heart conditions, undergone organ transplant surgery, suffer from keloid scarring, on any major blood thinning medication, allergic to metals, have any active viral infections or transmittable diseases. (Please contact me beforehand if you are unsure)

I authorise Jordana to carry out a Semi Permanent treatment on myself. If any issues arise, I authorise Jordana her full judgement to advise what will be best during my appointment.

I understand that I must not drink any alcohol or caffeine 24hr before my treatment!

I understand I must not take any aspirin, niacin, vitamin E, ibuprofen or any blood thinning medication 48 hours before my treatment.

I understand numbing cream will be applied during my treatment, but I may still feel slight discomfort.

I understand that I am unable to bring extra guests (including children) to my appointment.

I understand I must keep my phone on silent during my appointment.

I understand different skin types and lifestyles may influence the outcome of my treatment and may need 3 sessions to perfect my brows/lips.

I understand if I have had previous pigment, I must send photos to Jordana before booking. If I do not send photos and continue to book with Jordana and she is unable to carry out the treatment I forfeit my deposit.

I understand SPMU is a process and requires top ups, therefore the healed colour is not guaranteed.

I understand additional work can only be done 6-8 weeks after my original treatment.

I understand that if I have/had an MRI or CAT scan I will inform my doctors and will present a doctor’s note before continuing with my appointment.

I understand if I'm taking any medication, I must disclose this with Jordana and present a doctor's note allowing me to go ahead with the treatment.

I accept full responsibility for the shape, colour and positioning of my brows/lips as agreed during my appointment.

I agree to follow all pre and post care information provided by Jordana. I understand infection and scarring may occur if I do not follow the correct aftercare instructions given by Jordana.

I accept all responsibilities for the result of my treatment. If I do not allow Jordana to continue I must still make full payment.

I understand all deposits are non-refundable under any circumstances.

I understand If I cancel or do not attend my appointment for ANY reason, I will forfeit my deposit and I will need to make a new booking and pay a new deposit.

I understand deposits cannot be transferred, if I decided to cancel or amend a booking.

I understand if I arrive late to my appointment and I'm unable to be seen, my appointment will be cancelled. If I would like to reschedule my appointment there will be a £30 rescheduling fee added to my balance.

I understand I’m able to reschedule 48 hours before my appointment once, before I forfeit my deposit. I will then need to make a new booking and pay a new deposit.

I understand if I would like to reschedule within the 48 hours of the rescheduling period, there will be a £30 rescheduling fee applied to my remaining balance.

I understand it's my responsibility to book my top ups in the correct recommended time to avoid any additional charges that will be applied.

I understand if I do not attend my top up appointment for any reason. I will forfeit my deposit and will need to pay a new deposit for a new booking. I understand my top up price will change according to the length of time of my new appointment.

I understand if I have purchased an offer treatment including a Top up and I do not attend my top up appointment for any reason, an additional fee will be applied according to the length of time of my new appointment.

I understand if I miss 2 top up appointments for any reason, I will be blacklisted and will not be able to rebook in with Jordana.

I understand no waxing or tinting will take place 3 days before my treatment and Jordana will shape my brows for me during my appointment.

I understand I must not use retinoids/Retin-A, anti-aging/acne creams or serums containing acids over my brows as this will fade them permanently.

I understand I must not undergo any chemical peels 60 days before or after my treatment.

I understand no Botox treatments should take place 3 weeks prior to my brow appointment or 4 weeks after my treatment.

I understand I must stay out of the sun for up to 14 days during my healing period.

I understand I will need to complete a consent form on the day of my appointment.
